(“Terasen Gas”) Rate Schedule 36 including Appendix “C” - Notice of Appointment of Marketer standard form; and B. On April 27, 2004, Terasen Gas proposed amendments to the format of the Notice of Appointment of Marketer form contained within Rate Schedule 36 to enhance the usability and enforceability by both marketers and customers. NOW THEREFORE pursuant to Section 71.1(6) of the Utilities Commission Act, the Commission approves for Terasen Gas its May 4, 2004 Application for Amendments to Rate Schedule 36 – Appendix “C” Notice of Appointment of Marketers standard form, effective immediately: 1. The font size should be reduced so that the entire form fits on page 32 of Rate Schedule 36. 2. Additional spaces should be provided for the “Title” of the customer signing the document as well as the name and signature of a witness. The signature line and title line should be repositioned. The witness information is to be identified as optional. 3. The header, “Terasen Gas Rate Schedule 36” and footer reference should be removed. 4. The marketer will be allowed to brand the standard form with the marketer’s logo or related identity provided that the marketer does not otherwise alter the appearance or content of the document. . . ./2
